Help Problems when charging s2?

Hey guys and girls. I made a recent post about the 'charging paused - voltage too high' problem as well as my phone lighting up with the battery charged notification every time it's been charged, and this lasts for hours sometimes.

Anyway, just now my S2 has started bleeping and saying the voltage is too high again, and i had it plugged into my pc via the USB lead at the time (so i can count out the fact my wall charger is causing it... right?)

I thought i would be clever and clean up the battery terminals incase that was the problem, then thought why not just measure the battery voltage with my electrical meter. I measured the voltage with the setting on DC upto 20V and it said 4.00 volts exactly - and before i removed it from the phone it said while it was charging on the lock screen "charging 77%". So if my battery is at 4 volts when it's at 77% no wonder it tells me the voltage is too high, the battery is only a 3.7 volts!

Can anyone advise me if the battery is the problem, so i can just replace it? I've had the phone on contract since june 2011 so its a little over a year old and currently i dont have insurance for it, as it was to expensive. I am hoping this is something for my provider (o2) or samsung to pay up for, as i have been nagging at o2 for a new battery for months as it has never held more then a single days charge.

I have read the solved threads of the voltage too high problem, but none of the actual ways of solving it are relevant to me.

I havent recently updated any software, or had the phone ANYWHERE near water, or anything at all unusual to everyday use. This message litterally begun out of no where and i have no explanation why - i have had to charge the battery every night as it is always flat by the evening of the next day. If it's because its been charged too often, then that is samsung's fault as the battery is very poor at holding charge
 
if for example a battery is 3.7 volts, to charge it needs more like 4-5V DC. though it shouldn't be doing that, most of usb port gives out 5V which is exactly the same voltage as the samsung charger only difference is the amperage, which is 200mA less and will charge the phone slower. i recommend that charging should be done with the original charger and using usb only when really needed.
