Help Rooted but need to return - what to do?

I've rooted my Evo when I first got it. Over the winter I was getting weird stuff happening to my Evo...any time I would put it in my jacket pocket (and now sometimes my pocket - seems like if it has room to move around and rub the touch screen on soft clothe...) it resets itself. So I'm wanting to get a new one. I haven't changed the Rom on it so it's still stock, just rooted with some programs requiring root.

How should I go about doing this?

Also, side question, I keep getting an OTA update that keeps popping up and I thought I remembered seeing a post a while ago about not updating if you've rooted. I just wanted to make sure that I should continue to click cancel on it.

While I might suggest searching google, as you could have got your answer faster, here is the easiest and fastest way to unroot:

NEVER accept OTA updates when you are rooted. Period.
