Root [rooted] In-call headset volume too loud

I have an issue where my in-call volume is easy too loud when using earbuds or a bluetooth headset. The audio sounds as if it can blow out the speakers. The in-call volume won't go below 1 during a call and adjusting it seems to have no effect.

It hasn't always been like this but I've removed all apps that could have caused it. Any recommendations? I've already tried some suggestions on moving the volume all the way up and back down.
 
What version of android and sense are you on? I seem to remember at some point this started happening on my device.
 
This issue only happens about 90% of the time. I have no idea what is causing it because it will work fine on one call then mess up on the next 20. Guess I'll try removing a few more apps. I really don't feel like doing a restore.
 
I think the only way I was able to do anything about it was to make sure that my media volume was low before i made a call
 
I had my headphones plugged in during this last call and was blaring. All volume was set to low with Volume Ace before the call. I muted everything during the call and it had no affect. Checked all settings and made sure hearing aid was off but made no difference if it was on. Not sure what to do now. :'(
