Help s3 wont lock

I am having problems with my s3, it will not lock, I click the lock button and the screen blacks out but the phone doesnt lock, I press the lock key with the screen black anf it pops up thr dcreen I was on last so the phone never actually locks, ive done master reset and all the troubleshooting I can think of but the problem still exists. I also get a black rext box with yello w writing which ill post a pic of, if anyone has any ideas outside of sending it in to samsung please let them fly

Have you actually tried changing the lock options in the phones security setting ?

Like to a pattern or a pin ?

Yes I have, ive tried each of the settings, when I press the lock button after the screen blacks out, its a black text box with yellow text, its phone and OS info in the box

What is the model number and OS running?

T999 and newest jellybean update 4.11 I believe, I work for tmobile and ive never seen this issue before
