Phone never "rooted"

OK, hoping the above is enough detail on my device.

Problem: Up until around the last OTA to 4.3 (eta March 8) my phone would charge on numerous cables around my house, vehicle, and office without any issues.

Since the update my phone has had repeated problems on each of these previously unproblematic locations. Some cables are connected to USB ports off my PC or laptop, some are cables into wall outlet chargers, some are in our vehicles.

The phone reports (using Battery Widget app) that it is charging, health is good, and level is xx% from time I plugged it in to charge.

And yet it is actually discharging over time. Slowly, but surely, discharging. It matters not if it is plugged into different USB ports or the wall. In fact, last night, plugged into wall at the bedside, it depleted and died while I slept!

Thinking the OEM Samsung was getting tired and not holding charges as long, I had purchased 2 additional new Li-ion batteries a few months back and been able to rotate my batteries leaving the most depleted to charge in a wall charge; let me go cable-less for longer. Each battery has this problem however. Yet they do charge when in the wall charger outside the phone.

Also, I have noticed that when plugged into a cable (whether USB or wall power) I often get the disconnect/connect sound from the phone, that occurs each time you plug it into a source even though it has not moved or been touched. Sometimes it is repetitive and non-stop; only seems to stop if I wiggle the cord or phone. Other times it stops on it's own after one or a few times.

I am at a complete loss. Each cable worked in the past flawlessly. Each USB port (granted they can fail) worked flawlessly. Each outlet charger worked flawlessly. Now they each have a problem.

Could it be the OTA update? Any ideas?
