Help SD card not recognized

Okay out of the blue my incredible quits recognizing my sd card. I can get to recovery and see the files there, but the phone is not recognizing it when I use programs ( Titanium Backup is the first one I tried). I did reformat the SD card and still the same problem. Is it a bad sd card or a bad reader in the phone?? Thanks for the help

Most likely a bad SD card. that said the only way to test is to either put the SD in another phone/reader. Or put another SD card in the Dinc...
