How do I get my phone to reconize my SD card

Normally put it in. Reboot the phone. Format the sd card (if it's new)
Menu button
System settings
Storage
Format sd card - note that this will wipe everything from the card!
If this does not work try it in other devices. If it wont work in any its probably dead.
