Help SD files disappear after a while

I have a Samsung Galaxy Note 4 - going to get the Note 10 later this year, but until then, I want the SD card to work correctly in my Note 4. This issue only started a few weeks ago - otherwise it had been working fine for about a year. When I take the card out of the phone and put it in my computer, it works 100% - no issues. I did a chkdsk on it, no errors. It NEVER fails in the computer and all my files are there. With the SD card in the phone, once the files disappear, the only way to bring them back is to go to Settings, Storage, SD Card (which shows as being there), and tap Unmount. The card unmounts without issue, then I click SD card again and it mounts it - no issue. Then I tap Format and format the card - no issues. At that point for about the next 5 minutes or so I can access everything on my SD card - all the files are there, even though I just "formatted" the card. I can view the photos, share them, transfer them, everything - no problem. But after a few minutes, everything is "gone" again and I have to go through the entire process over again - very annoying. Obviously SOMETHING is causing a glitch with how the phone sees the mounted space - but only after several minutes have gone by. Has anyone had this issue and solved it?

just to be sure, have you tried other cards? you might want to try a factory reset as well. my note 3 and note 4 never had this issue. i sold my note 4m but i just discovered my note 3. i might try and root today or tomorrow just for $hit$ and giggles
 
While not exactly the same, I had some similar SD card issues. Mine would suddenly just disappear, as if it had been unmounted. I could shut down, pull the card and re-insert it, and it would work for anywhere from minutes to days... then do the same thing over again.
The only fix i found was to remove the SD card, and insert it in a trash can. Apparently, my card suddenly went bad... no rhyme or reason as to why, it just did. In trying it with other devices, including 2 different DJI drones, it would also act up in varying ways.
I'd try another [high quality] SD card and see if the problem still exists. If so, you might be looking at an issue with the card slot itself, or some other hardware failure (firmware could be flaky too, but not as likely).
Btw, after you upgrade to the new Note, shout if you're interested in selling your 4, especially if its in decent shape cosmetically. I absolutely love mine, and wouldn't mind having a spare hanging around. Considering I left mine on top of my pickup a couple of days ago and drove off, I'm lucky to be typing this on it to start with. FTR, the only damage was to the front of my folio wallet case and a small nick to the bezel above the selfie cam. Definitely a tough lil phablet to hit the pavement from 6+ feet up at ~40+ mph! Thank goodness for honest people that pay attention to things in the road, or it would have been a goner. 😭
