Help Sending in broken ultra after insurance claim

Returning my ultra w/ broken screen, have the new one in hand.

How do i unroot and wipe this device clean of all data and preferences before dropping it in the mail?
 
How bad is the screen broken? If it's to where you can't get around on the phone, then there's no need to unroot, as insurance/Samsung will have to completely re-flash the phone and install a new screen before they can do anything with it.
 
I gave Sprint my phone rooted after it stopped charging and they took it without mentioning it. The guy scrolled through my apps and everything.
