That's really weird. I don't know what to say - perhaps you have an app for which you granted permission to control the volume? I know someone who had the free ad-supported version of Text Twist 2 on a Galaxy Nexus and a Galaxy Tab 2 - on both devices, even when in silent mode, some of the ads would play with volume enabled.
