TWRP not installing custom ROMs?

I've tried using different versions of TWRP, currently on v2.7.1.0 as I thought the newer versions may be causing problems. Whenever I flash a ROM (be it Lineage, CM13, or CM14 Nightly), I get a 'failed' message in TWRP. What is causing this and how do I fix it?

Update the recovery. Are you doing a factory reset before or after flashing the rom? Reset in Twrp not the settings.

Are you flashing roms for the correct model?

You do not say which of the 42 different models of Galaxy S5 you have. See...

(Some carrier minority variants have carrier mega locked down bootloaders that prevent rooting, custom recoveries or custom ROMs)

The latest version of TWRP is 3.1.0-1 for most Galaxy S5 models. See...

Official CyanogenMod is no longer supported. Lineage has replaced it.
