Help Unknown icon in Notification bar

Greetings,

Using a Nexus 4 with Android 5.1.1 has been going well, and another Titanium Backup occurred successfully this morning. I have yet to upload it to Google Drive.

There is an unknown icon in the notification bar this morning, with no clues as to its significance. It is a rectangle taller than wide, with 2 side wing components. Pulling down the notification provides no information or icon, and swiping up restores the icon.

After power cycling and killing as many services as possible, Settings -> Apps -> Running shows only Settings, Google Play Services and Keyboard, yet the unknown new icon is still there in the Notification bar.

Any ideas about what is providing this icon, please?

Your phone is on vibrate. Press the power button to wake your phone, then press either volume down or up, and you should see the vibrate icon like in the screenshot below. Tap the the icon to toggle vibrate/sound on.

Hah! Thanks ... not sure how the volume got silenced, but I'm somewhat embarrassed to say that I never noticed that icon before, even though I've had the phone silenced during seminars and the like.
