Help UNLOCKING ISSUE OR BAD/WRONG SIM CARD?

I am switching a Galaxy S5 from Boost to T-Mobile.
When I received sim card. Their 3-in-1 nano card. I called T-Mobile to activate... Gave them IMEI, etc.. Phone ok to be actived. They informed me I'd have dual service until completed
2 hrs later.. Still not activated... Message says invalid SIM.
I took out and reintered, even used adapter and same thing.
It never asked me for a unlock code or stated phone is restriced. I restarted phone.
I called tmobile again.. They said the wrong sim card was sent... but now that I look at receipt from last order.. their sending me the same card. When I called again to inform.. that agent even noticed that and said I'd need a Micro not a nano..
He also asked if the phone was unlocked. Boost said t-mobile can do that.. but T-Mobile says its boost that does that..
When I try to make a call it says sprint cannot complete call..
Now im wondering if its just the sim or both. When I go to settings about device... network state is disconnected.. network is boost.. does that mean its unlocked..
and how do I unlock it? or check to see if it is?

Only the original carrier can give you the unlock code.
The 3-in-1 includes adapters for micro and standard sim slots.
Just remove the outer standard adapter, not the inner micro adapter.

Thanks. I have the code but nowhere is there a screen to type it in...
I have used it with the adapter... and still doesn't work.
Attached is card ... (middle is microsd card for size comparison).
Friend told me his is side of black adapter.. however even with the sim in adapter it does not work .. slot still reads boost card fine..

If boost Mobile uses tmobile's network, you will need a different carrier's sim card to unlock. Try borrowing an AT&T sim.

Hey i have a virgin moblie phone could i put my boost mobile simcard inside. If so what is the sim card code an how do i do it please
