Help Unnamed Folder

For some strange reason my phone Icon on the homescreen has a big grey box around it and it say " Unnamed Folder ", i don't know how it got that way but does anyone know the fix for this ?

I have to push the folder twice just to make a call.

Tap & hold the folder,then slide up to the trash can,just as you would with a widget.
Then replace the phone icon where desired,just as you would w/any app.

Or,open the folder,press & hold the phone icon,move out of folder & place somewhere else on the screen.
Then,the folder should disappear,provided there's nothing else in the folder.
Then place the phone icon/shortcut back into place.
