Being a newbie at Android, I upgraded my memory card from a 4 gig to a 16 gig yesterday and discovered this problem. If you have moved any apps to the memory card from the phone ( I always thought they moved to the non- removable SD card rather than the ext SD card, but apparently some components of the moved apps are on the ext sd), and then you uninstall the ext card, some of your apps are also removed with it , even though you copied all the folders on the old memory card to the new one.
So after I started up the phone again, I saw that about 8 of my apps had disappeared along with their shortcuts , and I would have needed to re-install them all. So I had to put the original SD in the phone, move all the apps one by one back to the phone back to the original sd card using the apps menu, and then reinstall the new card. Lesson learned.
