User Forms

I am currently trying to create a user form that my department can fill out while conducting a survey and I've run into a huge problem, I have no idea how to go about doing this. In short, what I want is a data input form that will post the data entered into specific cells in a spreadsheet on the same device (meaning that the device that is using the form doesn't need an internet connection) and will continue to enter the data until the survey is complete. I guess what I want is more of a macro than a form because once the first row of information is entered the form will have to restart the process with the row below and so on and so forth. I have seen this done on an Apple iPad 2 and I would like to know how to do it on the droid system (I REFUSE to buy an Apple iPad). Can anyone help?

Yeah I have but the form that they have is not the style that I need.

You know you can customize the google docs forms to do anything or am I missing something?
