Root [Verizon] Rooted Two S3's - Got Two Different Screens

Ok, so I rooted my S3 about a month ago using the ODIN and flashed the samsung ROM with full bloat. Everything went well and was able to do a back up with CWM.

A few days ago I rooted my wife's S3 using the same method...everything went well but the start up screen is different (Shows an unlocked lock and says "custom") and when I try to do a CWM backup it fails and says there is an "error with the bootloader image".

Any ideas what happened?

was her phone running the new baseband/firmware out of the box?
 
I would say bad flash; wipe it and start completely over. Ensure the ROM passes MD5 check in Odin.
