Help Volume keeps reducing automatically

I often find that the ring volume has reduced by itself, causing me to potentially miss incoming calls. This seems to happen randomly, and without any indication such as the "silent" symbol. Any idea why this happens and any solutions? I have even tried a factory reset without success. Thanks in advance for any suggestions.
 
If it's 'random', or at least you can't see any specific pattern, could it be that how you hold it, carry it or mount it accidentally touches the volume buttons? I have this issue when I use my phone in my wife's car. The way it sits in the console, unless I'm careful, will touch the vol. down button against the armrest and sets my volume to 0. Drives me batty.
 
A couple other things to look for would be gestures or events that might automatically mute it. Some phones when you flip them over will set them to silent. It might be helpful if you told us which phone you had.
 
Is it just the ringer volume? Does the problem go away temporarily if you restart the phone?

I had an issue where my phone would lower the volume of everything because it would always think that I was in the middle of a call. The problem turned out to be the way a messaging app interacted with Wi-Fi calling.
