Volume keeps turning down

For some odd reason, I will miss a call or a text and wonder why I didn't hear the ring. So, I go into the settings and see that the ring setting is turned nearly all the way down, so I crank it back up again. Then, later it is back down again.

What could cause this? Is there some action which will cause it to automatically turn down the volume, not just silence it for a while, but actually adjust the setting down?

Do you carry the phone in your pocket? I wonder if the volume button on the side is getting pushed? My phone will go into the music player if you hold down the side camera button for 3 seconds when it's closed and sometimes it gets pushed while in my pocket. Do the side buttons get locked when the screen locks? If it's not that, there might be an app on there that's causing the problem.
