Help Weird Ringtone Issue, Help Solve My Mystery

First thing first, I'm not crazy. Now for the issue. I have a noticed that 3-4 times a month my phone will change the ringtone for an incoming call by itself. It only happens for one call and not with the same contact. Here's the weird part, the ringtone it changes to can not be found on my phone. The phone is stock from factory and I don't have any ringtones assigned to any contacts.

It's always the same ringtone, but I've gone through my list (settings, sound, incoming calls) and none of them are what plays . I'm more puzzled by not finding the ringtone than why the phone is changing it. Any ideas?

I have the exact same issue, so it's not just your phone. Still no resolution on how to fix it...

This happens occasionally when I turn the phone off to charge it. I just go in and check/re-set the ring tones where necessary. Hopefully, ICS will fix this SOON.

I have this same issue. As others have said, it happens fairly randomly.
