Help Weird Voice

So with in the last week my phone has started to say at random times that is having troubles generating reports and to try again later. Has anyone had this issue before or know what may be causing it?

At random times? As in, it matters not what you are doing with your device, there is an audible notification that states that?
Hmm, odd indeed!
What device/carrier/Android version do you have? Is the device rooted?
Has it had a recent OTA update?
