What does this In-Call icon mean?

This icon shows up during a call with a specific person. I found the icon in the phone.apk application. It is called "call_birthday_icon.png" ... but what exactly does it mean when this gets displayed?

I get that figure back I am on 3G (which agency Video Calling is available) AND I accept forwarded my calls to addition number. As anon as I abolish alarm forwarding, the figure disappears. This figure is additionally not there back I am on Edge and alarm forwarding is enabled.

I don't think that is my case. I don't video chat and don't know what alarm forwarding is.
