Not to get all that way about it, but...
If this is your real master's dissertation, I would fix a few problems with the questionnaire now.
1st, master's in what?
2nd, you dissertation questionnaire comes off as a market gimmick.
3rd, your forms are easily bugged out. You can cause a few questions like the problem one to give a false reading. If they click no problems, they still can add info to the other fields.
4th, and here is the biggie. Any official dissertation questionnaire that was done at any school that I attend, it was a requirement to have the full name of the student, full name of the school, the department, the supervisor, and full address on the front of the questionnaire and the footer of the questionnaire.
Why, because in the past this has been used by less then scrupulous individuals for profit marketing schemes.

No problem, just want to make this work out for you. I had to do 3 dissertation, masters, doctorate, and post doctorate. They almost killed me, so I understand that you are trying to focus on the wasp nests and have very little time for the nagging individual wasps.
