Root Can't wipe/restore in CWM; make_ext4fs failed

So I've been having issues when trying to wipe data and restore factory settings in CWM. I (think) I've followed the instructions to root/install cwm/flash Broken Out and haven't messed anything up...But something is wrong somewhere (likely my fault)... the message I get is:

make_extf4fs failed on /dev/block/mmcblk1p4

I've been able to restore to a factory image (downloaded from here), but it still gives me that message afterwards if I try to wipe.

Any ideas or solutions?

My only recommendation for you is to flash CWM from the app itself now that the Connect is listed in the app's device list. It takes about 20 seconds to do so. :)
 
I get the same message when I try to restore using CWM. Does it have something to do with the fact my MicroSD card isn't partitioned?

No. Surprisingly to me, it doesn't seem like many people around here do partition.
