Root Are these signs my phone has been flashed before?

Hi,

I'm looking for some advice if anyone can help.
Recently I suspected my phone may have been flashed without my knowledge for malicious purposes.

Can anyone tell me if these occurrences can confirm that the phone has in fact been flashed.
Phone: Galaxy S4 GT-I9506 running Android 4.3 purchased new from Telstra Australia.

1. When the phone is on "Factory Mode" is a running app. Through some searching I came to the conclusion that this occurs when a phone has been flashed and it loses some files which means it runs in factory mode after?

2. When the phone is booted into recovery mode the following errors are displayed;

Code:

E:missing bitmap icon_att_fota_installing
(Code -1)
E:missing bitmap icon_att_fota_updating
(Code -1)
E:missing bitmap icon_att_fota_complete
(Code -1)
E:missing bitmap icon_att_fota_error
(Code -1)


I was reading through some forums and apparently this error can occur with ClockworkMod installation?

Any help greatly appreciated.

The phone is trying to update on at&t ota(over the air).And failing.

Well my guess is you bought it used,
Yes it could have very well been flashed,It could have been on AT&T at one time because its trying to update AT&T updates.

That's strange. I bought it new from Telstra in Australia and have never rooted, flashed or done anything other then a factory reset to the phone. I believe the phone was flashed by a hacker to infect it with malware so I really need to know.
