Help My android phone charges extremely slow

Okay this problem is really serious, my android phone charges really slow, not only that but, when i use it while charges my battery decreases. ive tried many ways, i turned off the wifi, bluthooth i even turned on airplane mode, screen brightness to the lowest and when i turn it off it still charges really slow, i have installed some apps that can help me like DU battery saver but it dosent work. i cant seem to find the reason why, i tried different chargers, i tried different electrical plugs( i dont know what theyre called). please help me and id be very thankful if it workss

DU Battery Saver doesn't help, it's adding to the problem.

What kind of phone is it? Is it old enough for the battery to be dead?

Not charging right when turned off would likely be a battery fault.

Post back some screen shots of app usage from GSam Battery Monitor.

If it was in a fast boot mode rather than off and not charging properly, GSam may be helpful in learning what's up.

Where does the OP states that CWM is installed, or indeed that the device is even rooted?

Something might be wrong with the 'fast' charging after an update. I'm had that problem after flashing a custom rom. Using battery widget app I found that it never charged >400mah. Unplugging the battery from a Nexus7 might've solved a problem when it was not charging past 66%.
