Root 128 GB Micro SD card not recognized by phone

I recently got the San Disk 128 GB micro SD card for my Warp Sequent. I was upgrading from a 64 GB card that worked fine. They are both Ultra SDXC cards but I can't seem to get my phone to recognize the 128 GB card. It is recognized on my computer and I can save files on to it from the computer.

I tried to format it on the phone thru CWM 6.0. It will start formatting, get hung up and then the phone will restart. Any suggestions? Any help y'all can give would be appreciated.
 
Im sure the phone just won't recognize that much storage.
That's why phones say "expandable storage up to 32 or 64gb using an SD card".
My phone can't recognize a 64gb.
Another I have can't recognize 32gb.
It just depends.
Most likely a 2014 phone read it.

Are you serious, you had a 64gb card on it and worked? if so consider yourself lucky cause as far as i know the Sequent is only expandable up to 32gb :)
