Well, the sun finally came back out today, and my problem has mysteriously vanished. I've not done any resets, etc since noting the issue a few days ago. I did install several other camera apps to test, so maybe one of those somehow made things better? But I only tested with the stock S4 app today, and it's working great again (thank goodness).

A never rooted user has the best chances of a hassle free update, even electing not to factory reset (but it's still a good practice).
Those who have had problems with the Sprint S4 may have done one or several of the below scenarios:
- Believed a simple click of the "unroot" option in SuperSU would restore them to stock. It doesn't and the integrity of system files is still permanently changed. This causes a glitch or failure of the update.
- Left a mod installed thinking everything would be okay. It isn't and resulted in all that bad stuff previously mentioned.
- Tried to restore their 4.2.2 ROM over 4.3. It's a one way trip. Trying to go back will soft brick your phone until you install a "4.3 Knox bootloader" based ROM again.
- Tried to restore apps/data to 4.3 "knox protected" System folder before TiBu released their update yesterday supporting SELinux. This caused programs to lockup and even boot loops or soft bricks.
If you are rooted on MF9, you have to reinstall the stock MF9 ROM via Odin to make the phone "like new" before doing the update, be it OTA or manually downloaded. The MJA ROM still hasn't surfaced, so anyone who borks their phone has little choice on what they can do right now to fix it.
As Darnell mentioned, a non-rooted phone has the best chance of a hassle free update. So go ahead and do it once you get the system notification. Should be very smooth.
The problems have not been isolated to one carrier. Carrier is not really the issue, it's rooted users not following specific upgrade paths. In hindsight, my mistake was thinking the MJA install had not gone correctly, when it actually had. I was missing the "mobile data" button on the system icons curtain, so I thought something had gone wrong. Turns out, that is just missing on MJA. So I caused myself a lot of pain for nothing. Doh!

If installing 4.3 is an update/upgrade for NOT ROOTED phones, why is it necessary to perform a "hard reset" which wipes phone and data to restore phone to original factory settings? I understand to need on rooted phones - where there is a nandroid to restore the phones' settings, apps and data, but there is no equivalent for NOT Rooted phones. Do Samsung and the carriers want owners to root there phones to have nandroid capability? Does the fact that so many people require factory resets following this "update?" de facto suggests that they are advocating and promoting the rooting of the Galaxy S4s!

Well I finally got the 4.3 update today over two weeks later than expected. I'm not sure why they changed the 4G icon.
I had to "trick" my phone by deleting app data for Google Services Framework, but then I found and installed it.
I've noticed 2 things thus far:
1. I get the "an unatuhorized application has tried to access your device" whenever I connect to WIFI. I've confirmed this is ONLY when I connect to WIFI, and per other posts on here it is related to Knox. I'm backing up my data and will try factory data reset to see if this clears it up.
2. Another weird thing I noticed is after the install was complete, on my first home screen, I noticed app icons overlaid on top of one another. So it put an app icon directly over the top of an icon that was already in that spot. I've noticed this a few times now and this never happened prior to the update. Again, hopefully factory data reset fixes this too.

Yeah, now it says 4G LTE. They changed the 3G icon too so it is the same font and style as the 4G icon (which I think is better than having 2 different styled icons).
