Address For Rebate

Got my droid last night at my local Verizon store.

Since I bought it at the Verizon store I need to do the rebate thing.

I'm going through all my receipts and stuff and for the life of me I can't find the address where I'm supposed to mail the sticker and receipt to get the rebate.

Does anybody know it or where I can get it?

It was in the Mail In Quick Rebate Form, don't know how I missed it before.

And yes that is the exact same address I have.
