Root App data between different phones

Can app data be transferred to a completely different phone? I love the Incredible, but eventually it will be replaced. If the new phone is rooted, I'd love if at least my saved game data could transfer.
 
Just a guess but between (1) moving the SD card over and (2) titanium backup I imagine you would be covered. Never done it though. Good question.
