App to show whether incoming calls are a forwarded call?

I recently started using the call forwarding feature of my pantech 'dumb phone' to shoot the calls over to my warp sequent...

But I can't tell if a call is coming from the pantech or my zte. Anyone here know of an app that will tell you in a popup or the like to show the call is someone dialing the pantech?
