App updates question

Id noticed recently that ill update an app then a few days or even weeks later ill get another update notification and the "whats new" seems to be the same as the last one. At first i thought it was just lazy devs but then i started taking note of the version number and release date and it turns out, they were the exact same update as id done before so <1> is even possible for an app to "lose" the update they got or <2> is it more likely Play just thinks im still on the older version <3> another?
P.s, most of my apps are Titanium restored if that makes a difference

From a dev viewpoint, we have 2 versions eg. my app currently is


android:versionCode="7"
android:versionName="0.43"

The version code is an incremental increase and the version name is the one you see.

I did once in error try to upload an apk with the same details as the previous version but the play store bounced it as being already published.

Added: I'm not sure whether it would be possible to change just the version code.
