Help Application Failed install to SD and/or Move to SD

Can't Install Apps from Market to SD card or Move currently installed Apps to SD Card. SD Card works for general file transfers and has loads of space. Apps that were on the SD card are showing as not installed with default Android icon in menu but they still there on the SD card when I check via usb mass storage connection to pc. This came after my phone crashed and switched itself off during usb connection to pc now I can't do anything app wise to the SD Card. Phone Not Rooted.

Found out my microSD card has some how been write protected it won't write to the card or format the card but it reads fine. Any Suggestions would be appreciated. Tried an external SD adaptor with tape to fix the lock switch but still protected. Is there a utility to force format or similar?

I moved apps to the SD card, and now want to move them back to the phone - when I tap "Transfer to phone" I get the message "Appl. cannot be found.". There is plenty of free space both on the phone and the Sd card. Any suggestions?
