Help Audio Failure, corrupt OS?

justabout

Lurker
I purchased my One S a few months ago and everything worked perfectly for a while.
I'm not sure if my current issue is hardware or software related. I have not rooted the phone, I have not installed any non-market apps.

Basically, the audio has started to stop working at random. When the audio cuts out, trying to use anything that would have made a sound will either lock up the phone temporarily or cause it to reboot.

I have wiped the phone back to factory several times. When i go into the boot menu, it says ***Tampered*** on the top, not sure why.

Even after resetting, I get "Process system is not responding, etc etc" before the phone even reaches the OS. If I choose to wait it hangs there for minutes and then the phone operates normally (sometimes minus the sound).

The sound will work perfectly when it wants, other times it will cut out immediately, whether on speaker or headphones.

How can I go about putting a new OS image on the phone in case this one is corrupted somehow? Is this possibly hardware related? Thanks.
