I have a Verizon Samsung SCH-I545 I lost root on my phone from an OTA update, and had to get back in. I found taht there was a version of Lollipop available to easily load while keeping root, or regaining root. My wife also has one, so i was able to test my steps, and refine them on two phones.
SO i hope these will help someone. You welcome to use them as base for another tutorial as well, citing this post would be helpful if you do that.

These steps are for your SCH-i545 ONLY, and if you lost root, to either
a) Accidentally going to NK4, , but not going all the way to lollipop,
or
b) going from anything, to lollipop rooted.
FINAL ACTUAL STEPS

SKIP TO STEP 5 if you already have ROOT
1) Download the following files, and extract:
a) this SCH-I545_VZW_1_20140330160240_8il17ls9o2_fac.zip
b) I545_OC1_Stock_Rooted_ROM_wNK4_BL.zip https://www.androidfilehost.com/?fid=23991606952602601
b) download odin version http://www.droidviews.com/download-odin-tool-for-samsung-galaxy-devices-all-versions/
c) FlashFire_v0.18.apk
d) Towelroot-v3.apk
e) I545_OC1_TetherAddOn.zip https://www.androidfilehost.com/?fid=23991606952601552
2) run ODIN follow procedure here:http://www.droidviews.com/install-stock-firmware-on-samsung-galaxy-devices-using-odin/
a) SYNC all google contacts, BACKUP APPS and DATA
b) download odin, and EXTRACT it to your computer, where you do your mobile phone work for this particular phone, also extract your 8il17ls9o2_fac.zip image here too
c) enable usb debugging, RESTART PHONE into donwload mode, DONT PULL BATTERY, see below
d) reboot into Bootloader/Fastboot(vol dwn + home, followed by pwr), press up vol once you see the message
e) look for com port, and added in odin, then continue
f) press AP, then navigate to ALL_VZW_I545VRUFNC5_I545VZWFNC5_1151670_REV06_user_low_ship_MULTI_CERT.tar.md5 which is the unzipped SCH-I545_VZW_1_20140330160240_8il17ls9o2_fac.zip
g) Make sure only auto reboot, and F.Reset Time are checked then
e) MAKE SURE ALL above requirements are met in last step, then PRESS Start,
f) wait for success message on ODIN, then exit odin as phone reboots
YOUR flashed, let phone reboot and finish updating.
3) copy files:
a) once rebooted, connected via usb, copy files to your phone now
1) Towelroot-v3.apk
2) FlashFire_v0.18.apk
3) I545_OC1_Stock_Rooted_ROM_wNK4_BL.zip
4) I545_OC1_TetherAddOn.zip
5) eu.chainfire.supersu-2.46-246-Android-2.1.apk
b) go through intial setup, skipping everything possible.
4) Root and gain Super User access:
a) open file explorer in app tray, browse to where you copied towelroot.apk
b) install towerlroot by clicking on it
c) installing, open your apptray, look for towelroot, run this app 'make it rain'
d) copy and install eu.chainfire.supersu-2.46-246-Android-2.1.apk, or get it from supersu playstore
e) install supersu, let it update (TRYING NOT DISABLING KNOX on SHARONS. ON MINE, disabling knox)
f) Open SuperSu Free and check off 'enable su during boot'
g) install checkroot
e) verify root using checkroot
START HERE IF YOU ALREADY HAVE ROOT

5) Set up system for upgrade:
a) enable usb debugging
b) install flashfire by opening 'my files' in app tray
6) Flash Lollipop with Flashfire:
a) open flashfire and grant
b) click +, then choose Flash ZIP or OTA
c) browse to I545_OC1_Stock_Rooted_ROM_wNK4_BL.zip
d) make sure automount isnt checked, then click checkmark
e) now click + agaion to create another action
f) action is WIPE, select all BUT internal storage
g) press checkmark to continue
h) reorder the two actions so that wipe is first
i) when done, tap lightning bolt
7) prepare for enabling tether, install flashfire again, if its not:
a) after reboot, sign in to google account, (connect to your wifi right away)let it update your apps and contacts
b) open 'my files' install flashfire again, this will prompt you to allow unknown sources, allow this.
c) open root checker and check root, allow grant
d) Open SuperSu Free and check off 'enable su during boot'
8) Enable tether:
a) open flashfire and grant
b) click + select Flash Zip or OTA
c) select the G900A_OC4_TetherAddOn.zip that was already copied to the phone
d)make sure automount isnt checked, then click checkmark
e) when done, tap lightning bolt
f) let it reboot
