Help Battery and USB charging issues

I've had my most recent DNA since June 13', and while I've never been thrilled with the batttery life, about 3 weeks ago I started having issues with my battery draining much faster than normal. In one instance I lost 15% of battery from less than 5 minutes of browsing my facebook newsfeed and then lost another 10% for a 15minute call that would usually only use about 2%. Simultaneously I also started having problems charging my phone from my work PC via USB. After 3 hrs I'd only get a 5-6% boost at most. I haven't done anything to the phone besides powering on and off, restarting and a soft reset on the day it started happening. Charging via my original wall charger seemed a tad slower than normal but not to the point of being a nuisance. Oddly, a couple days ago my battery starting giving me my normal usage again and since yesterday I am able to charge via my work PC's usb port with no problems anymore. Any ideas as to what would have caused the problem for nearly 3 weeks that has apparently cleared itself up?
 
And today the battery drain problem returns. From 7am-5:30pm with less than 30 min of usage (no streaming audio or video, just facebook, words with friends and a few websites) and charging via USB for 4.5hrs I still lost 50% of my battery.
 
Anyone have an idea what is going on? I am having the same issue although mine started yesterday. Plugged into my PC for about 3 hours and it went up 3%. Lost 10% power in 10 minutes on idle. I checked my battery usage and it showed Google Maps was taking 78% even though I wasn't using it. Forced stop and app remained on the list. I uninstalled the updates and it finally disappeared. Now, "Android System" shows it is taking up 74% but I have no idea if that is right. Is it normal to be that high? Google search shows people are around 7% up to 40% but mine is at 74...
 
Have you tried a new usb cable? For whatever reason, they can go bad and cause all sorts of symptoms.

You can always try Helium Backup, backup all you can, do a factory data reset (that clears your apps and data but more importantly clears the hidden operating system caches) and then do a restore - that often works wonders in these cases.

Finally you can try GSam Battery Monitor, check around over a few days, see what's still sucking power.
 
Tried a total of three cables and they all yielded the same results. I guessed it's time for a new battery. Ordered one and will attempt to swap it out. Factory resetted the DNA and re-activated my old Incredible 2 (no 4G). Used the DNA for about 30 minutes of web browsing and left it on standby for 11 hours and it went down 38%.

Tried GSam Battery Monitor and nothing was sucking a lot of power but it did show my battery had enough power to last a little over 4.5 hours (at 100%).
 
I've tried multiple USB cables as well. Today I got it to start charging normally via USB for about 2 hrs and then it started discharged a few percent while charging with the same cable. I'm 3 months away from an upgrade and just trying to make it limp along till then.
 
I swapped out the battery last night and in doing so, the touch screen doesn't work anymore. I'm 5 months away from an upgrade and WISHED I have left it alone. I'm now stuck with my Incredible 2 without the 4G and am crawling along.

While on the Incredible 2, my DNA was left in standby but I paid attention to the battery lost...
Friday 1/9 - lost 38% in about 11 hours
Saturday 1/10 - lost 42% in same amount
Sunday 1/11 - lost 30% in same amount
Monday 1/12 - lost 28%.
