Battery has charge but dies

My phone is charged completely. After a couple minutes of being on battery power, it switches to 0% and shuts off. As soon as it does I can turn my phone on, which will indicate the 100% battery charge. I can stay in Clockworkmod all day, infact I've wiped everything, recovered from a restore point, and wiped all user data all on battery power. It seems it's only in the software that it's randomly getting a 0% reading from the battery. Anyone know of a fix for this? Thank you in advance.
 
battery could be failing, cwm takes a lot less power

edit: I believe battery % is based on voltage
 
Take battery out and see if it easily spins on a hard, flat surface. i bet on one side it spins like a top. Battery is most likely fried. They do wear out. I'm on just my second one and the phone has been used hard for 2+ years. This one has to be at the end of it's life.
