Help Battery, sensor or something else ?

I have a problem with my phones battery level indicator.
Most of the time now after a full charge, it will indicate full charge regardless of how long the phone is on. Rebooting the phone will fix it until it is charged full again. I have had the phone turn off because of a depleted battery a couple times now without a low battery warning, this is most likely the cause.

A little history, It worked fine when I got the phone, however, I discovered the wall charger was defective and would make the touch screen almost unusable while it was plugged in (linked to a similar issue), but worked fine on USB or with my battery pack. My belief is that it was unstable or too much power getting through. The charger has since been replaced.

Could this have damaged the sensor or battery or is there something else I could be looking at to solve this indicator problem ? I did a quick google search and found nothing dealing with this.
