Boost mobile 1st sim card

It's a UICC card...evidently all LTE phones need them now...

What is a UICC and how is it different from a SIM card? | Just ask Gemalto

Correct.

The phone is still a CDMA phone. Not GSM.

GSM and CDMA phones both use the same 4G LTE frequency. So CDMA phones do require a UICC card for 4G to work. Notice on a CDMA phone, you can use the phone without the UICC card and it will still get 3G. Not so with a GSM phone. On a GSM phone you wouldn't be able to do anything except call 911.
