Help Broken black lock screen "widget"

It's my first time here so I hope I did everything correctly :P

So here's my problem.
Every time I end a call, the following "widget" appears on my lock screen and I cant get rid of it.

Its the black box at the top.

I have two apps that also create similar widgets when they play, those are Tune In and Rdio but once I close them, the widget they create goes away while this one is kind of stuck, I cant remove it. Usually I should have the Dashclock widget there (previously I had the default clock but even then this happened).

My device is Nexus 4 and the version of android is 4.3

I hope someone knows what this is or why it happens

I have a Samsung Galaxy S4, and it's doing the same thing. I think it has something to do with Google. After I updated my apps, it's been giving me all sorts of problems. Battery life has decreased significantly, too.
 
Try the app "NotCallLog". You can designate what screen you want your phone to go to when you hang up. I got it so that when I end a call I'm on the Home screen, not the Phone screen, but it might help your problem.

I feel the NotCallLog is kind of a workaround, like healing the symptom not the cause :/

I tried disabling and enabling the lock screen, that didnt help :(
