DX $100 Rebate - Form?

I'm a new VZW customer (currently AT&T) and just ordered a DX which is supposed to ship around Aug. 3. I paid the $300 for the phone and got a register receipt for the payment. But I forgot to ask about the rebate form. Does a form come in the box when they ship the DX to me? Or does the form get generated when I activate after getting the DX?

Form comes in the box. Please make a back up copy of all paperwork submitted and be mindful of the time frame to get it in.

For future reference, ordering online involved an instant rebate instead of mailing in the rebate form.
