EVO Icon?

Well I searched adn searched and couldn't find one so i tried to make one. I got a start but when I use this in bettercut/folderorganizer the icon is stretched. Can someone point me in the right direction or help in any way

make sure the dimensions are square.

The problem I am encountering is if I make the dimensions (for example) 128 x 128 it distorts the image. I have very basic knowledge of photoshop. Basically i am using resize tool and it ruins the proportions if i just use this tool and I don't know how to continue :/

EDIT:

I resized the canvas and made it square and it worked
