Root factory reset not working properly

so i just restored my x back to stock from apex. i unrooted the phone and performed a factory reset but when the phone came back i still had service (normally not the case after a factory reset). i tried doing this through the settings menu (i.e. privacy > factory reset) and the built in recovery console (i.e. holding home + power) but neither seem to do the trick. anybody know a method of getting a true factory reset, other than SBF? Also the phone periodically boots into ClockworkMod, which from what i under can be remedied by downloading the app and just bootstrapping it.

but id still like to figure out why the factory reset isn't doing what it should. thanks.
 
Did you make any changes to files in the System/Apps folder? If so, you probably need to get everything in there back to stock before the Factory Data Reset will work.
 
i did not knowingly modify anything in the system/apps folder. what are some immediate things that would affect this? all i did was install Apex ROM, fiddle around with it a bit and then restore back via Clockword Restore. Once restored i performed Factory Reset just for sh**s and giggles, but alas no dice.
