I'm pretty sure this is the one you're looking for. I threw it up on mediafire since i can never get attachments to work properly. Always get a database error on here.

Beautiful Widgets is only $2, and it's much better (customizable features) than what you are looking for.
I'm as frugal as the next guy, but this is the 1st app I bought when I left HTC Sense on the Eris.

Yeah, I use BW as well. I change it up all the time, but miss-placed my FW .apk.
