Help Hard reset - wiped internal SD

I've just upgraded my girlfriend's S2 to official OTA ICS, did a hard reset using the dialer code beforehand and to my disbelief, it has wiped her internal SD card.

Now I've definitely use the dialer code before to wipe mine and it left the SD untouched so what could I have done differently? Does using the dialer code usually do this?

That dialer code function is known as a factory format. You should have just done the normal factory reset. Not sure why your previous attempts didn't do it, unless you keep files on your external sd card on your phone so didn't notice the internal sd card getting wiped.
