Help How: Check if Google Voice/Sprint integration is active?

For some reason my text messages are now being sent with my GV# as the ph#. I had to wipe my phone and do a factory install and when I reinstalled the GV app, this started happening. How do I check to see if this is being caused by GV/Sprint integration? How do I revert back to my sprint number showing as the from # for sms?
