I think the easiest way would be to copy the entire contents to your computer and then paste it over to the new one. If you have an SD card reader on your computer it will be much quicker as the transfer rates are faster than using a USB cable. How much stuff do you have to copy over?
Open the old card, highlight everything and copy it exactly as is to the new card, I just went from 8gb to 32gb using this method and it worked flawlessly. All apps worked and all data for the apps was there. Kept my old card with everything on it just in case as a backup.

Don't mislead other people.
A simply copy will rarely work, because of differences between Windows and Android. Android has bizarre naming conventions for folders and files that will make Windows to abort the copying process.
