I have an HTC EVO 4G that has been flashed to Boost. I was not the one who flashed it, but is there anyway at all I can flash it back to Sprint? In other words, set the phone back to stock?

I am sure there is a way to do it, however, I don't know much about carrier flashes.
Is your phone rooted?

The funny part is, the phone is/ was rooted. I checked it today with root checker, and it says that it's not rooted.
I've been trying to root the phone with no success. In the bootloader it is not showing it is unlocked. But when I try to run "fastboot get_identifier_token" I get (bootloader) [ERR] Command error !!!

I did end up getting it to root. It was something I was doing wrong of course.

kolosus, do you know if there is any other way, other than using CDMA Workshop, to write the Sprint PRL back to the phone?

My understanding is that you can use dfs (google it, you can use the demo version) to flash the prl to the phone.
But if you're coming from Boost then you already have a Sprint PRL on the phone. The prl you have only has the Sprint tower info but it'll carry you long enough to get a Sprint prl on the phone.
