Your question could be understood a number of different ways, but I'll have a go at answering what I think you're asking:
You really cannot save your home-screen or launcher settings when you move to a new ROM. You'll have to re-setup your icons and widgets. This is a known annoyance when moving to a new ROM, or even flashing a newer version of the same ROM if a complete wipe is required.
If you use Titanium Backup to backup all your apps and data, you can restore your apps and data with all your settings by using Titanium Backup to restore.
Generally you'll have to 1) install your ROM, install Google Apps (Gapps); 2) reboot, let Market restore all your apps; 3) reboot, use Titanium Backup to restore apps. You probably should not use Titanium Backup to restore Google apps but, Google should restore your Google Apps settings that had been pushed up to Google, so that won't be a problem.
And always, you should take a full Nandroid backup before flashing a new ROM just in case you want to restore EVERYTHING as it was before flashing.
