Help Lock Screen issue?

Sorry, I am new to android. When I setup lock screen and turn off, the screen does not have the lock screen I setup when I turn it back on. It still show the swipe screen, no pin lock or password screen. Now if i power off and reboot then it show up. Also if I set up a type of pass lock then I can't use the lock widget anymore. It just won't show when I scroll right or left. Is my phone defected?

I noticed that my pin password is being turned off for some reason.

I set a 4 digit pin, it works a couple times then turns off.
